Bartenders, cocktail servers, or other persons solely engaged
in preparing or serving food or beverage.
Secretarial Personnel
Janitorial Personnel
Stage, sound, and light technicians
Other non-gaming personnel

2. FILL OUT YOUR APPLICATION
Items you must provide:
Application Forms (Completed & Signed)
Photo copy of official ID (Drivers license or state issued-identification
card)
Completed fingerprint card (in duplicate)
Photograph of applicant ( in duplicate)
$25 application fee for a one-year Gaming Work Permit (exclusive
of any additional costs)
